Registration & Payment: Please fill out the attached form and mail it or email it to Mandie Connell. If you need a separate invoice for golfing, let her know, she will be happy to create one for you. 

GOLF REGISTRATION FORM

Golf is limited to 48 golfers (12 foursomes). Foursomes will be assigned on an A-B-C-D level basis to ensure an equitable tournament. Requests for specific pairings will be considered but not guaranteed. 

Please let us know on the payment page if you will be joining us for dinner. We hope you do!


REGISTRATION POLICY:


Payment should be made in advance or brought with you to the seminar. Pre-registration is preferred. On-site registrations are accepted; however, there will be an additional fee of $10.00.

 

CANCELLATION POLICY:

 

Cancellation notices must be made in writing and may be emailed to info@nyfoa.org.

Cancellations made up to one week prior to the event, will be refunded the registration fee less an administrative fee of $15.00.

No refunds will be issued for cancellations made less than one week before the event; the registrant is liable for the full amount of registration.

No Shows are liable for all registration fees.
